We are Licensed as a residential service agency by the State of Maryland Department of Health, Office of Health Care Quality. Our agency is s owned and operated by a Registered Nurse who have over 25 years of experience in community and home health care . Our RN's , LPN's, CNA's, Certified Med Techs, Personal Care Aides and Companions are all professionals with experience and sensitivity to the needs of our aging population and children and young adults with disabilities and functional limitations. as well as disabled population.
Our essential health care team is carefully selected and screened, with clear criminal backgrounds. Each one is trained in client confidentiality, client bill of rights, personal health information, elderly/ child abuse and HIPPA Laws.
Each essential health care provider is CPR/First Aid trained and certified.
All skilled nursing is provided by MD state licensed RN's and LPN's.

We create an individualized care plan based on an Registered Nurse assessment of the client’s health conditions and or functional limitations, then customize it according to the client's needs (medical and or non medical), every client has and mantains an accurate "plan of service" or "plan of care ".
Coordinating Care with the Client, Physican and Representative
In developing the individualized Plan of Service , all factors will be considered and referred to the client, client's representatives and client’s physician if needed. The provision of your care is managed by and R.N., who will be available at all times to supervise personnel, assist with coordination and management of your services to help you achieve the best level of health care and home safety as possible.
Compliance with Standards
All staff members at Paradigm Home Health Care are CPR / First Aid certified, finger printed and have a clear Federal/ State Background check. TB negative and trained in Personal Home Care, Infections Control, Elderly/Child Abuse, and Privacy Health /HIPPA. licensed. The agency is also licensed by the State of Maryland, Department of Health, Office of Health Care Quality. We comply with applicable Federal and State Rules and Regulations.
Background and Reference Check
We are careful when hiring the people who will compose the team of medical and non-medical professionals at Paradigm Home Health Care. Prior to being hired, all personnel has to undergo thorough background and reference checks. This is a necessary step that we take in order to assemble a trustworthy team that will bring the brand of Paradigm Home Health Care to the homes that they will be assigned to work in.
To ensure that our clients receive easily accessible care, we require our staff to live within the 30-mile geographical radius of our service areas. In case of contingencies involving the inability of the scheduled health care providers to attend to his or her shift, a replacement can be immediately arranged. As much as possible, we will do what we can to reduce cases where the client’s schedule or accustomed living conditions are disrupted because care is not accessible from our agency. We will do what it takes for the continuity of care of our clients.
Family Members and Client Representatives are heard and considered
Family members will have an opinion of our care services – and we respect them for that.
